How StatePharm Rx collects, uses, and protects information on this website.

1. Who we are

StatePharm Rx ("we," "us," or "our") operates the website statepharmrx.com and related online services, including contact and refill forms and provider and patient portals (collectively, the "Site"). This Privacy Policy explains how we collect, use, disclose, and protect personal information when you visit or use the Site. It applies to website and account information. It does not replace our HIPAA Notice of Privacy Practices, which covers protected health information (PHI) used in pharmacy care. See our Notice of Privacy Practices for PHI.

2. Information we collect

We may collect: • Contact and identity information: name, email address, phone number, and mailing or shipping address when you submit a form, create an account, or request a refill. • Provider registration details: professional identifiers such as NPI, clinic or practice information, and related verification data needed to review portal access. • Account credentials: email and password (stored in hashed form) for login to applicable portals. • Communications: messages you send through contact forms, refill requests, or pharmacist messaging features, including any health-related details you choose to provide. • Technical and usage data: IP address, browser type, device information, pages viewed, referring URL, and approximate location derived from IP, collected through server logs and similar technologies. • Marketing preferences: email address and consent status if you opt in to newsletters or updates. We do not require you to provide more information than is reasonably needed for the purpose you are contacting us about.

3. How we use information

We use personal information to: • Respond to inquiries and provide customer service. • Process refill requests and coordinate with patients, caregivers, and prescribers as appropriate. • Create and manage provider or patient portal accounts and authenticate users. • Verify provider credentials and operate our pharmacy and portal services. • Send transactional messages (for example, account, security, or request confirmations). • Send marketing or educational communications when you have opted in, and honor opt-outs. • Improve Site performance, security, and usability; detect fraud or abuse; and comply with law. • Maintain records required for pharmacy operations, quality, and regulatory compliance.

4. How we share information

We do not sell your personal information for money. We may share information with: • Service providers that host our Site, store data, send email, provide security or rate-limiting, process payments (if applicable), or support shipping and logistics, under contracts that limit their use of the data. • Prescribers, clinics, caregivers, or other parties you identify when needed to fulfill a request or prescription-related service. • Professional advisers (for example, legal or compliance) under confidentiality obligations. • Authorities when required by law, regulation, court order, or to protect rights, safety, or security. • A successor entity in connection with a merger, acquisition, or similar transaction, subject to appropriate protections. The Site may use cookies, local storage, and similar technologies that are necessary for login sessions, security, and basic functionality. Retention periods for pharmacy and health records may be longer than those for ordinary website inquiries.

No method of transmission or storage is completely secure. Please use strong unique passwords and protect your login credentials.

8. Your privacy choices and rights

Depending on where you live, you may have rights to: • Request access to or a copy of personal information we hold about you. • Request correction of inaccurate information. • Request deletion, subject to legal or operational exceptions (including pharmacy record requirements). • Opt out of marketing emails (unsubscribe links are included in those messages). • Appeal a decision about a privacy request where required by applicable law. Texas residents and residents of other states with privacy laws may have additional rights. California residents may have rights under the CCPA/CPRA, including the right to know, delete, and correct, and to non-discrimination for exercising rights. We do not “sell” or “share” personal information for cross-context behavioral advertising as those terms are commonly defined, but if that changes we will update this Policy and provide required notices. 9. Children’s privacy

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. Parents or guardians who believe a child provided information may contact us to request deletion where appropriate.

10. Third-party links

The Site may link to third-party websites or services. Their privacy practices are governed by their own policies. We are not responsible for the content or practices of those sites.

11. Changes to this Policy

We may update this Privacy Policy from time to time. The effective date at the top of this page will be revised when changes are posted. Continued use of the Site after an update means you acknowledge the revised Policy.
